I researched 22 clinics across Medellin for TRT, HGH, peptides, and stem cells. Here’s the definitive ranking — from specialized clinics to hospitals to brokers — and the hybrid strategy that gives you the best of all worlds.
I live in Medellin, Colombia. I recently decided to get serious about biohacking — TRT, HGH peptides, stem cells, longevity protocols, the whole stack. The first question was obvious: where do I go?
I spent weeks researching. I ran three independent research rounds — deep web searches, clinic website analysis, review mining, regulatory checks. I found 22 providers across the spectrum: hospitals, specialized clinics, brokers, retreats, and everything in between.

Why Medellin?
Colombia ranks 22nd globally in WHO Healthcare Rankings — the highest in Latin America. But the landscape is a jungle. There are world-class hospitals, cutting-edge specialized clinics, brokers posing as clinics, and everything in between. Knowing the difference matters — because you're trusting these people with your endocrine system.
The Three Tiers
After researching 22+ providers, I found the landscape breaks cleanly into three tiers:
Tier 1: Hospital-grade institutions — Strongest medical governance, deepest diagnostics, best for baseline workup and safety. Weakest on biohacker branding and consumer speed.
Tier 2: Specialized clinics — Strongest service menus, peptide and stem cell depth, concierge experience. Variable on medical governance.

This is where you go to get a real diagnosis before anyone sells you a protocol.
HPTU won't design your biohacking protocol — that's not what they do. They'll give you the most medically rigorous baseline workup in the city. Comprehensive blood panels, specialist consultations, imaging. Take those results to a specialized clinic and you'll make much better decisions about what interventions actually make sense for your body.
#3 — Stem Cells Colombia
The most experienced team by procedure count — 8,000+ procedures under ultrasound and fluoroscopic guidance. Dedicated international patient office handles everything from appointments to airport pickup. Strong complement to any hormone protocol for repeatable, objective measurement.
#5 — TRT Colombia
Important classification: TRT Colombia's own disclaimer states they are "not doctors, clinics, pharmacies, nurses, or medical experts." They provide marketing and referral services, connecting clients with allied providers. This is a concierge and broker layer, not a clinical home.
That said, for what they are, they're excellent. Best English-first consumer experience in the market. Google 5.0 out of 5 from 73 reviews. Mobile nurses come to your home or hotel for blood draws and injections. VIP program with priority scheduling. Published pricing — TRT injections $425-575, pellets $800-1,200, HGH peptides $1,000-2,000 per month.
Use TRT Colombia as a convenience layer for logistics and English handholding. Not as a substitute for identifying your actual treating physician and verifying their credentials independently.
#6 — BioXcellerator
The premium stem cell brand. Most established in Medellin with a genuine GMP-certified onsite lab. Full white-glove concierge experience — clinic is inside the Novotel El Tesoro hotel, so treatment facility, hotel, shopping, and restaurants are all under one roof. All-inclusive packages from $19,000 to $41,000 include hotel, meals, and even tours.
The caveats: they received an FTC warning letter about COVID-19 stem cell treatment claims. Self-reported 80-90% positive results are not independently validated. Heavy celebrity endorsement marketing. And they're not a TRT or hormone optimization center — strictly regenerative. Consider only as a separate track after your hormone baseline is established.
#7 — XCells
Best social proof in the entire research — 4.8 out of 5 from 1,247 Google reviews. Most affordable stem cell pricing in Medellin: anti-aging from $1,500, joint regeneration from $2,000. Multi-country presence in Colombia, Panama, and Barbados. GMP manufacturing standards. Early-passage cells (P3).
One red flag: they claim "15+ years of excellence" but were founded in 2020. The 95% success rate is likely self-reported. Single lead physician mentioned. But the overwhelming review volume is hard to ignore.
The Rest of the Field
Reju Stem Cell Clinic (#8) — interesting for their results guarantee (free re-treatment if no measurable improvement in 6-8 months) and hospital-based setting inside Salud Vegas. But the anonymous "Dr. White" without a full name is a transparency concern.
Mountains of Hope (#9) — not a clinic but a 5-star residential biohacking retreat in Rionegro, 10 minutes from the airport. 35+ therapies, 7-21 day programs, 20-acre sanctuary at 7,000 feet elevation. Cold plunge, sauna, PEMF, neurofeedback, stem cells, organic meals. Perfect complement to clinical work.
Clinica Somos (#11) — the most credible direct TRT page I found in Medellin, with a named urologist (Dr. Esteban Gomez). More direct than a broker but thinner infrastructure than the hospitals.
Vitalitat Clinic (#12) — positioned as sexual health, not comprehensive hormone optimization. Named team, discreet private model. Keep on radar for ED or libido-specific care.
Cenegenics Bogota (#15) — the Colombian outpost of the Las Vegas-based elite anti-aging brand. 35,000+ global patients, 25% of whom are physicians. Worth the trip to Bogota if the brand appeals.
My Strategy: The Hybrid Approach
No single provider covers everything. Here's how I'm approaching it:
Phase 1 — Medical Baseline: HPTU executive checkup. Full blood panel, endocrinology, urology. Get a real diagnosis before anyone sells me anything.
Phase 2 — Performance Diagnostics: Las Americas Arkadia center. DEXA scan, ergoespirometry. Objective baseline measurements I can track over time.

What to Demand From Any Clinic
Before you hand your endocrine system to anyone, send them these questions. The clinic that answers crisply is the one taking medicine seriously:
Do you require two morning fasting testosterone tests before diagnosing TRT candidacy? Which baseline labs do you require? Who is the actual treating physician and what is their specialty? Can you provide their ReTHUS verification? Do you coordinate endocrinology and urology? What is your monitoring schedule for hematocrit, testosterone, estradiol, PSA, lipids, and liver markers? Which pharmacy supplies your medications? Do you use compounded peptides or off-label products? Can you integrate DEXA and cardiometabolic testing? Who manages adverse effects?
Also verify credentials through ReTHUS (Colombia's official physician registry), check INVIMA registration for any clinic handling stem cells, and note that INVIMA has issued public warnings about fraudulent testosterone pellets — product traceability is non-negotiable.
The Bottom Line
Medellin has world-class options for every aspect of biohacking. The infrastructure is here. The talent is here. The regulatory environment permits interventions that aren't available in the US. And the pricing is 40-70% lower.
But the landscape requires navigation. Hospital-grade safety and specialized clinic innovation don't come from the same place — and that's OK. The hybrid approach gives you the best of both worlds.
